Covid-19 Vaccination

McCrae Village Clinic is a COVID-19 vaccination provider  for both Pfizer & Astra-Zeneca vaccinations

Please check if you are currently eligible to get the coronavirus vaccine via our COVID-19 Vaccine Eligibility Checker.

This vaccine is provided free of charge to all eligible patients with a Medicare Card.

Patients without a Medicare card can be vaccinated for free at Commonwealth-funded GP-led Respiratory Clinics.

COVID-19 Vaccinations

We are offering COVID-19  to patients above 12 years with a valid Medicare card.If Booking Online, Please select the correct vaccination from the drop-down menu.

All patients existing or new will need to bring in a completed COVID-19 consent form to their appointment.

NEW PATIENTS 

If  you  are not  a regular patient of the clinic, you are required to attend the clinic prior to receiving the vaccine to complete the registration and medical history forms and hand over your completed COVID consent form.

This is to ensure that you have no contraindications to the vaccine and are registered in the system to avoid delays during the vaccination clinic.

To protect other patients, doctors & staff,

Please do not book a COVID vaccine appointment if you have • Fever, Respiratory, or any other symptoms suggestive of  COVID -10 • Visited a COVID Tier-1 or Tier -2 exposure site • Been advised to isolate • Are a Close contact of a COVID-19 .Are At-Risk Suspected COVID-19.
