GP chronic condition management plans (GPCCMPs)
We provide GP Chronic Condition Management Plans (GPCCMPs) for patients with long-term health conditions that require ongoing, structured care.
GPCCMPs support coordinated care planning and may include referrals to appropriate allied health and other healthcare providers, in line with Medicare and RACGP guidelines.
Reviews and ongoing care
GPCCMPs are reviewed regularly to ensure care remains appropriate and up to date. Reviews may be recommended at:

12 months
Additional reviews as clinically required
Eligibility, care planning and referrals are based on clinical assessment and Medicare requirements.
