New Patient Registration

All new patients are required to complete a new patient registration form and medical history form prior to their first appointment.

Contact & Emergency Information

It is essential that the practice is able to contact you or your nominated emergency contact if required for clinical or safety reasons.

Keeping Your Details Up to Date

Patients are responsible for ensuring their personal details are kept up to date with the practice, including address, contact phone numbers, next of kin details and Medicare information.

If your details are not current, the practice may be unable to contact you regarding abnormal results, recalls, reminders, or important follow-up care.

Test Results – Important Information

Patient Responsibility for Results

The practice maintains a policy requiring patients to take personal responsibility for their test results. Patients must ensure that appropriate follow-up is arranged with their referring doctor or specialist.

External Test Results

Test results that are ordered by external doctors or specialists and provided to this practice as copies are not the responsibility of this practice to follow up.

Responsibility for follow-up remains with the ordering practitioner and the patient.

Patients are advised to contact the requesting doctor or specialist directly to obtain and discuss their results.
